Understanding Your Choices: The key to receiving the right level of support begins with knowing your options. Our Care Managers serve as consultants, guiding you through the Home Care landscape. They listen to your needs and provide recommendations, equipping you to make informed decisions that affect your life.
Consultation: We meet you wherever is most convenient for you and your family. It can start with a phone call and continue at our offices or your home. Our goal is to gather as much information as possible about your daily routine and explore potential alternatives. If Home Care Support or Skilled Nursing is necessary and requested, our Care Managers prioritize the most relevant skills and attributes of Care Professionals to suit your needs.
Your Decision: You decide what services are right for you. Whether you need comprehensive care managed proactively by our Care Management or are not yet ready for Home Care Support, we guide you in the right direction, both now and in the future. If our Home Care Support is needed later, we can pick up right where we left off to ensure a smooth transition.
Care Professional or Nurse Selection: If you choose to move forward with our Home Care Services or Skilled Nursing, selecting the right Care Professional or Nurse is crucial. We prioritize your health and safety while also considering personality and common interests. At Kaji Care, we constantly review candidates for their skillset, experience, and passion for improving quality of life.
Start of Service: Once we identify a quality candidate, your Care Manager works with the Care Professional to ensure they have the right training. For Skilled Nursing, we ensure your doctor’s prescribed Plan of Treatment is clearly understood. Care Professional training includes condition and symptom-specific training, as well as client-specific training to truly understand your unique needs. Some of this training occurs onsite in your home, as Care Managers introduce Care Professionals to you and walk through aspects of your daily routine.
Ongoing Care Management: From here, the process truly begins. We learn from each other, adapting and making adjustments. Our Care Managers stay involved, following up with you and your network of Care Professionals to provide ongoing assistance, ensuring you receive the support you need.
Getting the right solution in place is a process. It involves focusing on the most important needs and making necessary adjustments. Kaji Care works exceptionally hard to proactively manage this process.
Situations where we excel:
Clients with Greater Needs or Preference for Higher Level of Service: We typically provide clients with 20 hours or more of service per week, although it can start with less.
